Community Health Worker III

Location: Dearborn, MI

Job Title: Community Health Worker III

Job Status: Full-time

Job Summary: Under general supervision the Community Health Worker III will conduct wellness consultations that include Social Determinants of Health screenings, health coaching, health education, and referrals to social services available in local communities that provide valuable assistance to residents. Evaluate the effectiveness of the services provided and their long-term impact on improving the overall health of the patients. They will also provide ongoing support to patients and their family members by providing referrals to Primary Care Physicians (PCP’s), community resources such as goods, transportation, housing, utilities, and medical insurance.

Essential Duties and Responsibilities:

  • Conduct social determinants of health (SDOH) screenings to identify individual or family needs
  • Refer clients to community resources and other agencies to meet identified needs
  • Conduct outreach in the community that promote, maintain, and improve individual and community health
  • Help clients to navigate and access community services and adopt healthy behaviors
  • Provide health education for chronic disease prevention (i.e.., blood pressure, cholesterol, diabetes)
  • Provide health screenings such as vital sign intake, HIV testing, Lead testing, and Point of Care testing
  • Provide smoking cessation opportunities with clinical pharmacist
  • Document details of engagement with clients
  • Conduct follow-up contacts with clients to determine if needs are met or additional support is needed
  • Assess client eligibility for services such as financial assistance, insurance, and other programs to assist the individual/family
  • Aid clients in obtaining healthcare services (Medicaid, family planning, connections to medical homes, etc.)
  • Completes (or closes loop) referrals by ensuring resources were accessed by patient
  • Serve as a liaison between communities, individuals, and service providers
  • Assist with recruiting new clients
  • Engage with community to identify referral partners and engage with existing referral agencies to strengthen partnerships
  • Participate in continuing education
  • Attend community meetings and health fairs to better understand community issues and provide information about agency offerings to community members
  • Provide ongoing remote support until social needs have been met
  • May work as part of the Mobile Health Unit
  • Operate standard office equipment and use required software applications
  • Perform other duties and responsibilities as assigned

Knowledge, Skills, and Abilities:

Knowledge of:

  • Social Determinants of Health and how they impact overall well being
  • Case management and/or care coordination
  • Local resources and agency programs that serve clients in the area of health promotion
  • Working in agencies that provide services related to community outreach, healthcare, managed care, public/community health, or health promotion/education
  • Providing health coaching and health education

Skill in:

  • Operating standard office equipment and using required software applications for program area and other applications, including Microsoft Office

Ability to:

  • Maintain high level of confidentiality and follow HIPAA standards to safeguard patient information
  • Work independently, problem solve, prioritize tasks, and adapt to complex situations
  • Strong desire to help others
  • Strong oral and written communication skills
  • Build rapport and trust with diverse populations

Educational/Previous Experience Requirements:

  • Minimum Degree Required:
    • High school diploma or GED equivalent

~and~

  • At least 3 to 4 years’ experience as a Community Health Worker and a track record of successful completion of all milestones and practical demonstration of performing tasks at a high level, or any equivalent combination of experience, education and/or training approved by Human Resources

Licenses/Certifications:

  • Licenses/Certifications Required at Date of Hire:
    • Current CHW Certification
    • Valid driver’s license and dependable transportation
    • Must successfully complete clinical research training, both in-person and virtually within 4 months of hire. Trainings include but not limited to:
      • CITI Modules
      • Motivational Interviewing
      • Implicit Bias
      • PAL2

Working Conditions:

Hours: Normal business hours, some additional hours may be required

Travel Required: Local, in-state, travel, up to 60%

Working Environment: Climate controlled office
